StageⅡ結腸癌治癒切除後再発高リスク因子の検討

はじめに

　Stage Ⅲ結腸癌では術後補助化学療法によって再発が抑制され，癌による死亡率が有意に低下することが明らかにされている^1,2）．これに対してStageⅡ結腸癌では術後補助療法の有効性が検証されておらず，術後の補助療法には消極的な見解が一般的である^3）．大腸癌研究会編のガイドライン^4）でも「StageⅡ結腸癌に対する術後補助療法の有用性は検証されていない．一方，再発リスクの高いStageⅡ結腸癌には術後補助療法を行う場合もある」と記載されており，stageⅡ結腸癌では再発高リスク因子を有する症例に限って治癒切除術後補助療法を行うというコンセンサスはある．しかし，再発高リスク因子の検討はまだ進んでいないのが現状である．

　今回，筆者らはstageⅡ結腸癌治癒切除後の再発高リスク因子について臨床的検討を行い，いくつかの知見を得ることができたので報告する．
