要旨：乳癌の標準的な治療を調べる際に，National Comprehensive Cancer Network（NCCN）に代表されるような臨床試験のエビデンスに基づいたきめの細かいガイドラインがネット上で閲覧できるようになった．乳癌術後の補助内分泌療法はホルモン感受性乳癌の治療の大原則である．様々な作用機序と薬物動態を有する薬剤を組み合わせた臨床試験の結果から日々エビデンスは蓄積されている．しかし，その結果を実地臨床にそのまま外挿してもよいか判断に困る場合もある．また，海外で推奨された治療法について，アジア人女性あるいは日本人女性のデータが乏しい点も気になるところであり，逐次更新されるガイドラインに振り回されている嫌いもある．本稿では最近の内分泌療法の知見を整理して，実地臨床における補助内分泌療法の現在を考えた．
