再発乳癌に対する内分泌療法

要旨：再発乳癌の治療においては全身療法で完治を目指すことは困難であり，効果に大きな差がなければ生活の質を第一に考えた治療法を選択するべきである．内分泌療法は副作用が軽微で，化学療法と比べて奏効率は低いものの奏効期間は長い．閉経前においてはLHRHアゴニスト＋タモキシフェンが一次治療として推奨され，二次治療としては卵巣機能抑制＋アロマターゼ阻害剤が有用である．閉経後ではアロマターゼ阻害剤が一時治療として推奨されるが，患者背景から抗エストロゲン剤も候補に挙がる．黄体ホルモン剤は三次治療以降に用いられる．内分泌療法の適応となる症例については内分泌療法から開始し，できるだけ本療法を長期間継続することが予後を延長せしめていると考えられる．
