切除可能であった腹腔鏡下腹会陰式直腸切断術後局所再発の1例

はじめに

　直腸癌に対し，術後の早期回復が期待できる手術術式として腹腔鏡下手術の有用性が報告されている^1）．しかし，中下部直腸癌に対する腹腔鏡下手術と開腹手術とを比較した研究は少数報告されているのみで，腹腔鏡下手術の安全性は十分に確立しているとはいえない．

　今回，前医にて進行直腸癌に対して術前放射線化学療法後に，腹腔鏡下腹会陰式直腸切断術が施行され，術後18か月後に局所再発をきたし，当院で骨盤内臓全摘術を施行して切除しえた1例を経験したので報告する．
