大腸癌多発肝転移に対する肝動注療法の現状と治療成績

要旨：大腸癌の予後を改善させるには，従来は切除することが不能とされた両葉・多発肝転移，いわゆるH3肝転移の予後を改善させる必要がある．肝動注療法は欧米では評価は低い．その理由は，全身化学療法と比べ奏効率は高いが生存期間に寄与せず，むしろ胆管狭窄や胃十二指腸潰瘍といった副作用が懸念されるためである．しかし，肝切除が唯一根治を望める治療法であることを考慮すると，H3肝転移に対して，まずneoadjuvant chemotherapyとして奏効率の高い肝動注療法を行い，さらに全身化学療法を加えdown stagingをはかってから肝切除を行うべきと考えられる．さらに，術後も残肝再発予防として全身化学療法を含めた肝動注療法を行い，治療成績の向上を目指さなければならない．
