多発肝転移に対する経皮的肝灌流化学療法の適応と治療成績

要旨：通常，多発肝転移については全身化学療法が主に選択されている．しかし，多発例であっても大腸癌の肝転移では外科的切除が可能な場合，筆者らは個数や手術回数に関係なく切除を第1選択としている．他方，胃癌や膵癌では切除することが不可能な両葉多発病変やリンパ節転移などの要因で肝切除の対象となる例は数少ないのが実情である．近年，大腸癌肝転移例についてはFOLFOXをはじめ有望な全身化学療法のレジメンが登場し，生存期間の延長が得られているが，有効率やQOL，予後などで限界がある．筆者らは経皮的肝灌流化学療法（PIHP）を開発し，これまで進行肝細胞癌を主な対象として治験を重ね，その成績を報告してきた．本稿では転移性肝癌に対するPIHPの治験成績を紹介し，今後に残された課題と展望について述べる．
