大腸癌肝転移に対するnew strategy

要旨：大腸癌肝転移に対しては従来から肝切除が最も予後良好な治療法であるが，診断時にすでに多発肝転移があるため，切除不能であることもしばしばある．近年，FOLFOXやFOLFIRIなどの全身化学療法や分子標的治療薬などの治療法が進歩し，高い奏効率に加え，生存期間の延長が認められるようになってきた．しかし，臨床的にCRと考えられる症例でも組織学的CRは一般に10未満であるため，さらなる予後向上のためには，術前化学療法によって肝転移巣が縮小した場合，肝切除を行うことが望ましい．われわれの教室では2005年から切除不能大腸癌肝転移症例に対してFOLFOXによる術前化学療法を行い，腫瘍が縮小し切除可能となった時点で肝切除を行っている．本稿では大腸癌肝転移に対する治療法とその成績を概説し，術前化学療法と肝切除による新しい治療戦略について述べる．
