内視鏡下手術

1 はじめに

　内視鏡下手術では主として硬性鏡を用いて，炭酸ガスで気腹された腹腔内や胸腔内でモニター画面を見ながら，特殊な鉗子，鋏，電気メスあるいは超音波凝固切開装置などを駆使して臓器の剝離，授動，切除や吻合・再建などの外科的手技が行われる．内視鏡下手術は低侵襲性であることや手術手技の技術的向上，術後のquality of life（QOL）改善によって，そして何よりも患者の満足度に支えられ，マスコミ報道を通じて社会的にも認知され，近年その手術件数が急激に増えている（図1）^1）．本稿では内視鏡下手術の歴史を振り返りながら，内視鏡下手術が従来の外科手術に及ぼした影響と内視鏡下手術の現況を述べてみたい．
