膵管内乳頭粘液性腫瘍に対する機能温存手術のメリット・デメリット

要旨：近年，膵小病変に対する様々な機能温存手術が開発され導入されている．しかし，技術的困難性があることや膵液瘻などの合併症が多いといった問題を有するため，その施行においては慎重な判断が求められている．また，進展度診断が不正確な場合には癌遺残による再発の問題が残される．しかし一方では，術後の膵機能温存やQOLの向上をはかれるといった利点も有する．術前の正確な進展度診断能の向上を目指し，適切な手術適応の決定を行うことで膵機能温存手術における問題を解決していくべきである．
