直腸肛門周囲膿瘍の保存治療と外科療法

要旨　直腸肛門周囲膿瘍に対する治療は抗生剤による保存治療と，切開排膿または痔瘻根治術による外科療法とがある．抗生剤の投与は膿瘍が完成していないごく初期や膿瘍の自壊後もしくは切開排膿後に補助的目的で行う以外に効果は期待できない．膿瘍形成が明らかであれば速やかな切開排膿およびドレナージが第一選択である．これまでの考えでは直腸肛門周囲膿瘍は痔瘻の急性期であり，いずれは痔瘻へ移行するとされ，診断がつけば一期的または二期的に痔瘻根治術を行うのが一般的であった．しかし最近，諸家の報告では直腸肛門周囲膿瘍の切開排膿後の再発率は平均35％程度とあり，これまでの概念を改める必要性を感じる結果である．肛門機能保全の意味からも早まった根治術は慎み，根治性を求めた効果的かつ速やかな切開処置を行うことが重要と考える．
