肛門管癌の診断と治療

要旨　肛門管癌は組織型が多彩で，組織型によって治療法が異なる．腺癌および粘液癌では手術が治療の中心で，わが国では直腸間膜内リンパ節の完全郭清と排尿機能や性機能に配慮した全自律神経温存側方リンパ節郭清が行われている．また腹会陰式直腸切断術が行われる機会が多いが，一部の施設では自然肛門機能温存を目指した内肛門括約筋温存手術も行われている．痔瘻癌では会陰創が広範な欠損となることが多く，皮弁による再建術も行われている．扁平上皮癌に対する欧米の標準治療はマイトマイシンCと5FUを用いた化学放射線治療であり，わが国では腹会陰式直腸切断術が行われる機会が多かったが，今後は化学放射線治療の割合が増加することが予想される．
