十二指腸ホルモン分泌と生理作用

要旨：十二指腸はcenter of gutと呼ばれ，多くの消化管ホルモンが存在する．十二指腸から分泌される消化管ホルモンはコレチストキニン，セクレチン，胃抑制ペプチド，モチリンの4種類であり，それぞれ特有な生理活性を有している．消化管ホルモンは脳腸ペプチドとも呼ばれ，その分泌や作用発現は神経系と密接に結びついている．消化管壁に存在する内分泌細胞は直接もしくは感覚神経からの刺激を受けてホルモンが血中または組織中に分泌され，遠隔の臓器に到達し，また神経を介してその作用を発現する．胃内容物の酸度や浸透圧，腸管平滑筋の緊張度，十二指腸内の酸度，栄養素の有無などの変化が感覚神経に伝わり，この神経を介して内分泌系に伝わる．消化器外科手術によって十二指腸が切除されたり，神経系の連続性が遮断されたり，脳神経が切除されたり，標的臓器が切除されれば，消化管機能は大きく障害される．十二指腸病変に対して外科的アプローチを行う際には，このような消化管ホルモンの知識が必要と考えられる．
