要旨：十二指腸第Ⅰ部切除術は十二指腸原発腫瘍などの十二指腸疾患あるいは胆囊癌十二指腸浸潤症例に対して施行される十二指腸機能温存および膵頭十二指腸切除の回避を目的とした縮小手術である．術式のコツとして（1）十二指腸と膵頭部との間の剝離操作を丁寧に行うこと，（2）切除断端と十二指腸乳頭部の位置関係を適切に把握すること，（3）後腹膜への固定状態や腸管断端の距離などから十二指腸吻合が困難なときは挙上空腸を用いて再建することなどが挙げられる．術後合併症としては縫合不全や吻合部狭窄などの腸管の切除吻合に伴うものに加えて，膵頭部の剝離操作による膵液漏などが発症し得る．病変の進展範囲をなるべく正確に術前に評価し，過不足ない切除を行うことが重要である．
