要旨：十二指腸乳頭部腺腫や早期癌を経験する頻度の増加や進展度診断の進歩を背景として，近年は様々な縮小手術が施行されるようになってきた．膵温存十二指腸分節切除術（pancreas-sparing duodenectomy：PSD）は膵と十二指腸の間が容易に剝離できるという解剖学的な特徴に基づき，膵切除を行うことなく膵管・胆管（共通管）を十二指腸壁外・膵外で切離し，十二指腸乳頭部を十二指腸とともに完全に切除する術式である．PSDは乳頭切除術より根治性が高く，幽門輪温存膵頭十二指腸切除術より侵襲の小さい術式と考えられる．症例の集積および長期予後の検討が必要であるが，適応を正しく行えばPSDは機能温存手術として位置づけられる．
