十二指腸乳頭部腺腫の治療方針

要旨：十二指腸乳頭部腫瘍に対する乳頭部切除は幽門輪温存膵頭十二指腸切除と比較して侵襲が小さく，適応を選べば完全切除が可能である．しかし，乳頭部腫瘍は術前の良・悪性診断や深達度診断が困難なことが多い．生検で十二指腸乳頭部腺腫と診断された場合にも画像診断や病理組織診断を総合して治療方針を立てる必要がある．また，良性腫瘍や粘膜内癌の場合は完全生検の位置づけとして乳頭部切除を行うこともある．われわれはこれまでに術前診断として，十二指腸乳頭部腺腫あるいはその疑いと診断された6例と臨床的に腫瘍が疑われるが生検標本の病理検査で腫瘍の所見を認めなかった1例に対して乳頭部切除を行った．最終病理診断は4例が腺腫，1例が乳頭炎で，2例が上皮内癌であった．十二指腸乳頭部腺腫と術前診断しても，切除標本による詳細な病理診断が必要である．
