食道癌に対するadjuvant/neoadjuvant chemotherapy

要旨：T1aを除くStage Ⅰ，Stage Ⅱ，T4を除くStage Ⅲの食道癌は根治手術の適応である．しかし食道癌は比較的早期よりリンパ行性転移や血行性転移を引き起こすため，術後の遠隔成績を向上させるためには補助療法が必要である．近年わが国から，食道癌の補助化学療法に関して2つの大規模RCT（ランダム化比較試験）が報告された．2003年に報告されたJCOG 9204臨床試験では，術後補助化学療法による再発予防効果が立証され，「食道癌診断・治療ガイドライン（2007年4月版）」では術後補助化学療法が推奨されている．続いて今年発表されたJCOG 9907臨床試験では術前化学療法が術後化学療法より有効であることが証明され，食道癌に対する手術補助化学療法の主流は術後から術前へと移行している．
