大腸癌に対する補助化学療法―欧米とわが国における現状

要旨：転移性大腸癌に対する化学療法の進歩に伴い，術後補助化学療法も大きな進歩を遂げている．欧米では，Stage Ⅲ結腸癌に対して，5-FU＋leucovorinに新たにオキサリプラチンを併用するレジメンが標準的治療となった．しかし大腸癌の補助療法については，欧米と日本とで比較すると手術内容，予後に違いがあるため，欧米でのエビデンスをそのまま日本にあてはめることには問題がある．わが国は，これまで欧米に比べて臨床試験で大きな遅れをとっていたが，あせらずわが国独自のエビデンスを構築していくことも必要である．幸いいくつかの大規模臨床試験が急ピッチで行われており，その結果によりわが国の大腸癌患者に自信を持って推奨できる治療法が確立されることを期待する．
