膵癌に対するadjuvant/neoadjuvant chemotherapy

要旨：膵癌の術後補助療法は有用であるとするエビデンスレベルの高いランダム化比較試験は少ない．米国では化学放射線療法を中心に補助療法が行われている．一方，欧州で行われた大規模な比較試験では化学放射線療法はむしろ弊害であるが，化学療法は有意に予後を改善させると結論づけられた．この結果から，欧州と日本ではgemcitabineによる化学療法を中心にして補助療法が行われている．術前補助療法には切除断端やリンパ節転移をより陰性化するなどの効果があると報告されているが，予後の改善に関するエビデンスはまだ得られていない．
