胸部食道癌切除術における高位胸腔内吻合の工夫

要旨：当科で標準としている胸部食道癌切除術における高位胸腔内吻合では，①大網動脈ならびに毛細血管網を温存した大彎側胃管の作成，②胸腔内アプローチによる可及的口側での食道切離，③大網動脈アーケードを温存した食道・胃大彎の器械吻合，④温存した大網による吻合部と気管剝離面の保護を重要なポイントとしている．本稿では，それぞれのポイントについて具体的な工夫を述べる．
