食道デルタ吻合の手技とコツ

要旨：われわれはlinear staplerを用いたfunctional end-to-end anastomosisの手技を応用した新しい食道胃管吻合（食道デルタ吻合）を行っているので，その手技とコツを中心に報告する．食道デルタ吻合は，まず頸部食道左断端の小孔と胃管大彎側後壁の小孔からlinear staplerを挿入し，徐々に縦隔側に向けながら食道と胃管の後壁をななめに合わせ，挟み込んでファイアすると，前壁縫合と後壁縫合が同時に完成する．あとはその挿入孔を横方向に2回に分けてlinear staplerで縫合閉鎖する方法である．食道デルタ吻合は簡便であり，かつ吻合口を大きくとることができる内翻縫合と外翻縫合を組み合わせた吻合法である．
