腹腔鏡下胃切除術における吻合のポイント

要旨：腹腔鏡下胃切除の普及に伴い，術式も多様化している．幽門側胃切除に加えて，噴門側胃切除，胃全摘なども各施設が施行するようになってきているが，腹腔鏡下胃手術におけるアキレス腱として，上部胃癌に対する噴門側胃切除，胃全摘における吻合部の合併症が挙げれる．腹腔鏡下では視野，術野にどうしても制限があり，開腹手術で使われている吻合器や縫合器の取り扱いは難しい．食道へのかがり縫い，自動吻合器のアンビルヘッドの挿入など難渋することも多く，縫合不全の原因となり得る．本稿では，腹腔鏡下胃切除における器械吻合の現況を中心に概説し，さらにわれわれの行っている安全なかがり縫い，自動吻合器のアンビルヘッドの挿入法を紹介する．
