消化管器械吻合における耐圧能の検討

はじめに

　消化管の器械吻合は1960年代にSteichen^1）が報告してから徐々に広がり，現在では各種の吻合方法が用いられている．器械吻合は簡便かつ安全であり，手術時間を短縮したり吻合困難と思われた症例を吻合可能にするなどの様々なメリットをもつ^2～5）．しかし，器械吻合でも縫合不全が認められ，その頻度は術式によって異なるものの2.1～23％と報告されている^6～9）．縫合不全はいったん発生すると患者の術後QOLを著しく低下させ，稀に致命的な合併症となることもある．

　器械吻合方法には様々なものがあるが，ただ経験と慣れだけで吻合方法を選択している場合も多く，適切な器械の選択，適正な吻合方法，漿膜筋層縫合の意義などには不明な点も多い．

　われわれはこれらの問題点を明らかにするために器械吻合に関する一連の動物実験を行い，3編にまとめ英文誌に報告した^10～12）．出版社の許可を得て，本稿ではこれらの実験のうち最も重要と考えられる吻合部強度に関する結果をまとめて報告する．
