要旨：甲状腺癌，特に乳頭癌は高頻度にリンパ節転移をきたすが，リンパ節転移が高度であることが必ずしも生命予後が悪いということにはならず，この点はほかの癌腫と大きく異なる．しかし，生命予後は良好でも，中央区域リンパ節の腫大をきたすと反回神経麻痺などQOLを著しく損ねる転帰をとることがある．術前画像検査によるリンパ節診断の精度は中央区域において低いため，画像診断の結果のいかんにかかわらず中央区域の予防的郭清をすべきであると考えられるが，一方，外側区域では術前画像検査の精度は比較的高く，特に超音波断層撮影を参考に郭清範囲を決定することが勧められる．術中のリンパ節転移診断の試みの1つであるセンチネルリンパ節生検は感度が低く，有用性は乏しいと考える．
