膵癌のリンパ節転移と治療戦略

要旨：難治癌と言われるもののなかでも，膵癌の切除成績は不良である．膵癌切除例の半数以上がリンパ節転移を伴っており，リンパ節転移の程度は膵癌切除後における重要な予後因子である．したがって，リンパ節転移高度例（N2～3，⑯リンパ節転移，転移個数≧4など）に対しては潔く放射線・化学療法に委ねるべきである．一方，軽度または陰性（N0～1，転移個数≦3）のように厳選された症例に対しては集学的治療を積極的に併用し，切除成績の向上をはかるべきである．そのための併用治療法の1つとして術前化学放射線治療があり，高い完遂率と確実な効果が期待できる．
