はじめに

　転移性肝癌や肝内胆管癌などの腺癌では，ときに下大静脈への浸潤を合併する症例が存在する．これらの症例に対して積極的に下大静脈合併切除を行うことは，その予後の向上に寄与する場合が少なからず存在することが報告されている^1～3）．下大静脈合併切除後の再建法には，下大静脈の切除範囲によって単純縫合閉鎖，パッチ再建，ゴアテックスグラフトによる再建，非再建の方法がある．

　本稿では，このうちリング付きゴアテックスによる再建法を詳述する．
