PTBD挿入留置のコツ

要旨：PTBD（percutaneous transhepatic biliary drainage）は閉塞性黄疸症例に対する減黄処置とともに，胆道疾患において診断と治療に有用な手技の1つである．施行前にMRCP（magnetic resonance cholangiopancreatography）やMDCT（multidetector CT）を行い，閉塞の原因や部位を同定して適切な胆道ドレナージを行うことが必要となる．近年，PTBDは器具の進歩や手技の標準化などによって合併症も少なくなってきているものの，いまだに致死的な合併症も起こり得るため，細心の注意が必要である．胆道疾患に対する理解を深めるとともに，胆管を含めた肝臓の解剖や生理を十分に理解したうえで，適切で確実な手技を習得することが重要である．さらに，PTBDの手技における「コツ」を理解し，合併症の早期発見および合併症率の低下に努める必要がある．
