胆管造影診断のコツ

要旨：胆管造影は胆管内腔の充盈像である．その本質は，ある程度の圧で注入することで胆管壁の線維化の差を胆管拡張の差として（段階的に）描出する動的な点にある．さらに，腫瘍表面と胆管内腔面の性状を空間分解能が高く描出し得る検査でもある．胆管像の読影には胆管の解剖，撮影技術，腫瘍の所見の3点の理解が必要である．肝内胆管の合流様式には変異が多く認められるので，これらの知識を十分に蓄える．そして，肝内胆管の立体構造を理解しながら腫瘍進展の範囲について，その肉眼型とともに胆管予定切離線を美麗に写真撮影する必要がある．腫瘍の所見は肉眼型に基づき浸潤型と限局型に分けて解析すると実用的である．
