予防抗菌薬の最近の話題―有効性と耐性化予防の観点から

要旨：従来，推奨されてきた予防抗菌薬の薬剤選択の妥当性について疑問を呈する知見が最近，出てきている．まず，膵頭十二指腸切除術で術前に胆道ドレナージを要する症例では胆汁中の菌種が変化しており，第一世代セファロスポリンの有効率は約30％にすぎない．術前ドレナージの有無もしくは胆汁培養結果を元にした薬剤選択によって良好な術後感染予防効果が得られる．また，大腸外科手術におけるターゲットの1つである嫌気性菌は，頻用されているセファマイシン系薬剤に耐性が進んでいる．カルバペネム系薬であるertapenemを使用した報告は薬剤選択に一石を投じるものである．耐性菌予防の観点からは腸内嫌気性菌の重要性が増している．耐性菌リザーバーとしての役割が指摘されており，嫌気性菌温存のための予防抗菌薬投与期間の短縮が望ましい．
