肝細胞癌の切除範囲を考える

要旨：慢性肝障害を伴う肝細胞癌の切除においては，根治性を高めるための広範切除はしばしば術後肝不全による手術死亡を惹起するため，機能温存はどの臓器よりもクリティカルな問題であり，根治性と安全性のバランスをとる必要がある．根治性という点では広範切除と系統的切除が理論的に優れているが，安全性という点ではlimited resectionが有利である．両者の切除成績に差がない場合もあるが，肝機能良好例では系統的切除は予後に寄与する可能性があり，原則として選択すべき術式である．肝機能不良な小肝癌に対しては肝切除範囲よりも残肝機能の維持を重視したlimited resectionが選択されるが，切除断端の癌遺残を防ぐ工夫が必要である．
