肺癌の切除範囲を考える

要旨：肺癌の根治手術は肺門・縦隔リンパ節郭清を伴う肺葉切除術が標準として普及している．しかし最近，診断技術や胸腔鏡下手術などの手術手技，周術期管理の進歩に加え，手術対象患者の高齢化や術後の質的生活（quality of life：QOL）の維持も重視されるようになり，癌の根治性を落とさずに肺切除範囲やリンパ節郭清範囲の縮小が試みられている．進行肺癌の手術では癌の根治を目指す拡大手術もなされている一方，局所進展やリンパ節転移のないことが証明される場合には，根治を前提にQOLも重視した縮小手術の可能性が検討されている．また，TNM病期分類の改訂時期も近くに迫り，標準的な肺癌手術の切除範囲についての考えは徐々に見直しがなされつつある．
