外傷による血腫との鑑別を要した良性後腹膜神経鞘腫の1例

はじめに

　神経鞘腫が後腹膜に発生することは稀であり，後腹膜原発腫瘍の5.5％を占めるにすぎないと報告されている^1）．後腹膜神経鞘腫の多くは良性であり，Whiteら^2）は自験例2例を含む57症例から得た標本の58検体中9例，15.5％に悪性所見を認めたと報告している．発生頻度が稀であるうえに特異的な症状に乏しく，画像診断でも神経鞘腫に特徴的と言われる所見はあっても特異性に欠けるため，後腹膜神経鞘腫の診断や良・悪性の判定は困難とされている^3,4）．

　今回，上腹部の打撲を契機として発症し，現病歴，超音波診断，CT，MRなどの画像所見では後腹膜血腫との鑑別が困難であり，血管造影によって腫瘍性病変と診断し得た良性後腹膜神経鞘腫の1例を経験したので報告する．
