肝癌の予後予測因子―CLIP score & JIS score

要旨：肝細胞癌患者の予後は腫瘍条件と肝予備能の両者で規定される点が特異的であり，（1）患者の予後を予測するため，あるいは（2）各国，各施設間の治療成績を比較するために腫瘍条件と肝予備能の両者を包含した統合的ステージングシステムが提唱されてきた．現在用いられている代表的な統合的ステージングシステムはイタリアから提唱されたCLIP（Cancer of the Liver Italian Program）scoreと，わが国から発信されたJIS（Japan Integrated Staging）scoreである．CLIP scoreは簡便で，幅広い患者群に適用することが可能であるが，層別化が均等でなく，真に予後のよい群，逆に不良である群が同定しにくい欠点がある．JIS scoreはCLIP scoreよりも層別化能に優れ，治療法の選択にも有用であるが，いまだworldwideな評価は受けておらず，今後の普及が期待される．
