慢性肝障害予後予測因子―MELD score

要旨：MELD scoreはもともとTIPSを施行する患者の予後予測因子として紹介された指標であるが，あらゆる慢性肝障害の予後予測因子として有用であることが示され，2002年2月に米国における移植肝配分の基準として採用された．MELD scoreの採用によって，移植成績自体を変えることなく待機期間中の患者死亡が減少し，平均待機時間も短縮された．一般的にMELD≧15がよい移植の適応と考えられ，わが国における生体肝移植適応の判断にも不可欠の指標となっている．術前MELD scoreが術後移植成績と相関するか否かはcontroversialであるが，MELD≧30のレシピエントに対する肝移植適応は慎重に判断すべきである．最近，MELDNaが紹介され，MELD以上に精度の高い肝疾患予後予測因子として期待が集まっている．
