膵頭十二指腸切除術後における合併症発生のリスク因子

要旨：膵頭部領域の腫瘍性疾患に対しては，一般的には膵頭十二指腸切除術もしくは幽門輪温存膵頭十二指腸切除術が適応とされる．その術後mortalityは近年は数％にまで改善してきたものの，morbidityはいまだに40％前後と高率である．そこで，当教室において経験した症例から術後合併症発生のリスク因子を解析するとともに，海外からの文献的報告を加えて検証した．その結果，“soft pancreas”であることが膵液瘻発生の最重要因子であった．しかしながら，morbidityをゼロにすることは不可能であり，合併症が発生した際に，いかに迅速・適切に対処できるかが重要である．
