はじめに

　転移性肝癌や肝内胆管癌をはじめとする腺癌などでは，ときに下大静脈や残肝の肝静脈への浸潤を合併する症例が存在する．これらの症例に対する積極的な合併切除再建は，その予後の向上に寄与する場合が少なからずある^1,2）．

　下大静脈の再建法としては，下大静脈の切除範囲により単純縫合閉鎖，パッチ再建，リング付きゴアテックスグラフトによる再建，非再建の方法がある．肝静脈再建は適応症例は限られるが，浸潤範囲が1/4周程度を超えており距離がある場合にはグラフト再建が必要になる．本稿ではこれらの再建法を詳述する．
