胃癌肝転移に対する治療戦略

要旨：胃癌肝転移の多くは全身病で，腹膜播種やリンパ節転移などほかの非治癒因子を伴う．予後は1年以内と不良であり，胃癌肝転移に対する基本戦略は切除不能胃癌に対する治療戦略を参照すればよい．現在，非治癒胃癌の標準的化学療法はTS-1＋CDDP療法である．胃癌肝転移に対する肝切除の有効性の評価は確立されていないが，例外的に大腸癌肝転移でしばしば経験するように，肝臓のみに病巣が限局している場合がある．症例を限定すれば肝切除後の5年生存率は20～40％程度である．特に（1）単発かつ（2）5cm未満，かつ（3）原発巣で漿膜浸潤がないという3つの条件を満たす患者が肝切除のよい適応であると考えられる．しかし，肝内再発が高率に発生し，より強力な全身化学療法が期待される．
