要旨：脾腫を伴った患者に対する腹腔鏡下脾臓摘出術は，偶発症や開腹移行が高率である．標準的な脾臓摘出術に習熟した施設が，今後，脾腫例に適応を拡大するうえで必要な手術手技，偶発症への対処法と周術期管理での注意点を概説する．
