診療ガイドラインの理解と実践

要旨：消化管間質腫瘍（gastrointestinal stromal tumor：GIST）の概念の変革とその治療の変遷に伴い，米国，欧州，カナダならびにわが国からGISTの診療・治療ガイドラインが提示されている．これらのガイドラインも，新しいエビデンスや治療法の確立に伴いすでに改訂が行われた．本稿では，わが国に多い検診で見つかる上部消化管の無症状の粘膜下腫瘍（GISTを含む）の診断と治療指針，これら小さいGISTないし粘膜下腫瘍への腹腔鏡下手術の適応と意義，進行GISTの予後改善を目指したアジュバント治療を含む集学的治療の現状，そして，イマチニブが導入され数年が経過した今，進行GIST患者を悩ますイマチニブ耐性GISTの診断と治療に関して，ガイドラインの理解を深めるとともに，最新の情報を紹介したい．
