GISTの病理組織診断と遺伝子診断

要旨：GISTは消化管に発生するKITを発現し，c-Kit遺伝子変異ないしはPDGFRA遺伝子変異を示す特殊な腫瘍である．大部分は肉眼的，組織学的および免疫組織化学的な病理組織診断によって容易に診断することができる．しかし，10％程度のGISTにKIT陰性ないし弱陽性を示す症例があり，このような症例では診断に苦慮する場合がある．また，稀にNF1や小児，傍神経節腫に合併するGISTの亜型が存在し，このようなGISTではc-Kit，PDGFRA遺伝子変異は存在しない．病理組織学的な診断だけでなく，患者背景，遺伝子診断を併用してGISTの組織亜型を正確に分類しておくことは，予後の推測，正しい治療の選択をするためにも重要と考えられる．
