要旨：外科的完全切除がGIST治療の第一選択であり，原発GISTの治療原則でもある．粘膜下腫瘍で腫瘍径が5cmを超えると悪性化の頻度が高く，手術の絶対的適応となる．5cm未満の腫瘍に対しては，増大傾向，超音波内視鏡で悪性を疑う所見，穿刺細胞診でGISTの診断確定のいずれかがあれば手術の適応となる．GISTの切除は肉眼的断端陰性を確保した局所切除でよく，リンパ節郭清の必要はない．偽被膜の損傷に注意し，術中操作によるruptureを避ける．切除後は腫瘍の悪性度を病理学的に評価し，5年以上のフォローアップを行う必要がある．切除不能例にはイマチニブの投与を検討する．
