イマチニブによる術前・術後補助化学療法

要旨：GISTに対するグリベック^®を使用した術前補助療法には問題点も多い．有効でない場合，逆に外科的切除の機会を失う可能性もある．なによりも，術前化学療法による効果や安全性は未だ直接的には証明されていない．術後補助療法の臨床試験としてはAmerican College of Surgical Oncology Group（ACOSOG）がZ9000とZ9001を行い，その結果が公表され，様々な議論を呼んでいる．Z9000試験の結果から，clinical malignant GISTに対しては術後補助療法に同意が得られるのではないかと思われる．Z9001試験の結果からは，腫瘍径6cm以上のGISTで，特にエクソン11変異のある症例では術後補助療法に意味があるといえそうであるが，これらの見解は傍証に基づくものであり，現時点では根拠に乏しいといわざるを得ない．
