イマチニブ耐性に対する局所療法

要旨：GISTのイマチニブ治療では，一部の奏効病変だけが増悪するものも多く，これは本治療の特徴ともいえる．このような限局性の増悪には，外科切除をはじめとする局所療法が有効な場合がある．エビデンス・レベルは低いものの，国内外の診療ガイドラインにもイマチニブ耐性の治療選択肢の1つとして示されている．耐性腫瘍の完全切除を行うことができれば，8か月程度の術後無増悪期間が期待できる．広範性の増悪は無増悪期間がきわめて短く，手術適応にならない．二次耐性腫瘍切除では長期無増悪が少ないことから，患者QOLを損ねる手術は推奨できない．耐性腫瘍切除後もイマチニブ投与が必要であり，術後合併症を作らないことが特に重要となる．
