抗凝固療法中の患者に対する痔核手術と後出血に関して

はじめに

　近年，心血管病変の予防や治療を目的とした抗凝固療法，抗血小板療法は日常診療のなかで一般的となっており，その治療中に薬剤の中断なしに外科的治療を行うと重大な出血を惹起する危険性が考えられる．一方，その危険を回避するために処置の前に抗凝固療法・抗血小板療法を中断したところ，血栓塞栓症が誘発された症例も報告されている^1）．したがって，最大の問題は血栓症の発症を予防しつつ，かつ出血性合併症を増加させないことである．しかし，抗凝固療法の中断の可否，またその時期や合併症の頻度などに関する報告は少なく^2,3），肛門手術に関しては皆無である．

　そこで今回，肛門手術のなかでも最もポピュラーな痔核結紮切除術を行った患者を対象とし，抗凝固療法の症例を中心に後出血の頻度などを検討した．
