Intersphincteric resection（ISR）手術の成績

要旨：超低位の直腸癌に対するintersphincteric resection（ISR）の手技と成績について概説した．本術式の手術時間や出血量は従来の超低位前方切除術と遜色ない．術後合併症では縫合不全率がやや高率であるが，一時的なストーマを造設するため全身的には大きな問題とならない．術後の肛門機能は内肛門括約筋の切除の程度によって異なるが，歯状線までの切除であるpartialもしくはsubtotal ISRではほぼ良好な肛門機能が温存され，術後の再発も従来の術式と遜色ない．長期の成績が十分に評価されていないため，現時点では本術式は超低位の直腸癌に対する標準術式とは言えないが，適応を厳密にして，緻密な手術手技で本領域や直腸肛門機能に経験のある医師が施行すれば，従来の低位前方切除術と同等な成績が得られると考えられる．
