胃癌胃全摘における脾摘出の功罪

要旨：胃全摘術における脾摘出の意義は完全な脾門部リンパ節郭清であり，外科医はそれによる患者予後の改善を期待してきた．しかし，現在までに脾摘出の患者予後に対する有用性を示したランダム化比較試験はない．むしろ脾摘出による免疫力の低下と，術中操作に伴う様々な合併症の増加が示されており，脾摘出に対する慎重な意見が多い．現在，JCOGにて大規模なランダム化比較試験が行われており，その結果が待たれる．
