クローン病に対する腹腔鏡下手術のメリットとデメリット

要旨：クローン病は年々増加傾向にある原因不明の慢性炎症性腸疾患であり，内科的治療でコントロールが困難な症例に対しては外科的治療が施される．比較的若年者に発症し，再燃によって複数回の手術を余儀なくされることが多いため，より低侵襲なアプローチ方法である腹腔鏡下手術の機会が増えている．近年は再手術症例や膿瘍・瘻孔を形成している症例などの腹腔鏡下手術の難易度が高い症例にも適応が拡大されて良好な成績が得られている．しかし，クローン病に対する外科的治療は根治的治療ではないことを肝に銘じるべきであり，クローン病の特徴や腹腔鏡下手術のメリット・デメリットを十分に理解したうえで治療戦略を立てることが重要である．
