大腸癌外科治療におけるrandomized controlled trial

要旨：大腸の手術法に関連するrandomized controlled trial（以下，RCT）の結果と評価について，①no-touch isolation technique（以下，NTIT），②器械吻合，③腹腔鏡下手術の3つに分けて概説する．①NTITは，日本の外科医の間では常識的な手技と考えられているが，外国のRCTではNTITのbenefitは限られたものであった．②器械吻合に関しては，初期の報告ではトラブルが多くみられたものの，器械の改善が進み，経験数も増加し，術中トラブルや術後合併症のリスクは低下した．③腹腔鏡下手術に関しては，短期成績として手術時間は長いが出血量が少なく，術後の痛みや合併症が少ないとされた．さらに再発率，死亡率などの長期成績は，腹腔鏡下手術と開腹手術で差はみられなかったとする報告が多い．RCTに優る評価法はないが，外科のRCTを試みる場合，外科手術が外科医個人の技術に大きく左右されるといった，手術に特有な問題点を考えておく必要がある．
