要旨：肺癌診療の効果的・効率的な診断・治療法を体系化するために肺癌診療の全領域に関する「EBMの手法による肺癌診療ガイドライン」が作成され，近年はこのガイドラインに沿った診断治療が行われるようになってきている．2003年に第1版が，そして2005年に改訂第2版が発刊された「EBMの手法による肺癌診療ガイドライン」は肺癌の診断，化学療法，放射線治療，外科治療，術前術後併用療法，中心型早期癌診断・治療などを含む6項目のほか，肺癌の組織型・病期別診療に関する項目が加えられている．本稿では，「肺癌診療ガイドライン」を臨床現場で活用する側から，外科治療を中心に本ガイドラインの有効活用法およびその問題点を述べ，また，現在進行中の改訂第3版に改訂・追加予定の項目などを解説する．
