GIST診療ガイドラインの有効活用法―ガイドラインに則したGISTの診断と治療指針

要旨：消化管間質腫瘍（gastrointestinal stromal tumor：GIST）は食道～直腸までの消化管と腸間膜に発生する，10万人に1～2人と比較的稀な腫瘍である．良・悪性の鑑別は病理組織学的にも困難であり，診断がつけば治療の対象となると考えられている．治療の第1選択は外科切除であり，5cm以下の比較的小さなGISTに対しては侵襲度の低い腹腔鏡下手術も行われ，その安全性，有効性が示されつつある．一方，進行・再発GISTには分子標的治療薬（イマチニブ）の臨床開発が行われ，その予後は画期的に改善された．特に，進行GISTに対してはアジュバント治療，ネオアジュバント治療やイマチニブ治療下の外科切除など，集学的治療によって予後の改善が期待されている．これに伴い，米国，欧州，そしてわが国でも一般医療者向けの診療ガイドラインの改訂が行われた．本稿では，GIST診療ガイドラインに基づいた最新のGISTの診断と治療を紹介する．
