肝胆膵外科手術後の乳糜腹水―生体肝移植術後に発症した乳糜腹水の1例

要旨：わが国は有数の生体肝移植先進国となり，その成績も良好である．しかし，術後合併症は避けがたく，適切な術後管理が必須となる．乳糜腹水は生体肝移植術後には稀な合併症であるが，当科でも1例経験したので報告する．症例は40歳の女性，原発性胆汁性肝硬変に対して生体肝移植術を施行した．術後より多量の腹水を認めたが，術後21日目より乳糜腹水を発症した．低脂肪食への変更，完全静脈栄養への変更でも改善を認めず，完全静脈栄養およびソマトスタチンアナログの併用にて速やかに乳糜腹水の消退を認め，術後166日目に軽快退院となった．生体肝移植術後の乳糜腹水に対して，完全静脈栄養およびソマトスタチンアナログの併用は非常に効果的であった．
